Sleep Disorder Clinic LocationsOur sleep clinic practice is offered through professional affiliations with physicians, hospitals and sleep centers within and outside the greater Fort Wayne, Indiana area.At present, we provide and accept referrals from locations in the states of Indiana and Michigan. You can have our office schedule an initial screening that's convenient to you, or we can recommend an experienced physician of sleep medicine, and/or a sleep study center in your community. Our Doctor is on staff at Cameron Memorial Hospital in Angola, Indiana, as well as being one of the Doctors on call to patients from the sleep center at the Community Health Center of Branch County in Coldwater, Michigan. Working with health care professionals in several locations, Doctor Urban's practice extends to about a 100 mile radius to other cities beyond our Fort Wayne, Indiana office. |
